BILLING INFORMATION DR. YERRINGTON
Full payment is due at the time of service and a Credit Card is required on file. We accept cash, Visa, MasterCard, and Discover. We also accept Care Credit. If our physician is contracted with your plan, the majority of members covered under this type of plan are still required to make some form of payment for service(s) rendered to them. This may be in the form of co-payment, deductible, or co-insurance. If your plan has a co-payment, your co-payment will be collected prior to being seen by the doctor. Co-payments, deductibles and co-insurance are requirements of your insurance plan and we are required under our contract with these plans to collect these amounts from you. Most members covered under HMO plans also owe co-payments. Co-payments will be collected prior to being seen by the doctor. We are required under our contract with these plans to collect these amounts from you.
BALANCES ON ACCOUNT
All previous balances are to be paid in full prior to additional services being rendered.
All cancellations and/or reschedules need to be done 24 hours before your given appointment.
If you are a patient cancelling for an IV Therapy appointment a $25 fee will be debited to your account.
If you are a patient canceling for a standard appointment a $45 fee will be debited to your account.
If you are a patient cancelling for a physical appointment a $50 fee will be debited to your account.
IV THERAPY PACKAGES
Our IV Packages consist of 3, 6, 12 months. Choosing a package allows for a particular discount. However, if you miss your appointment within the month given, your treatments do not roll over to the next month. If you must cancel your membership for whatever reason, you will be liable for 50% of the remaining balance of your package rate. Credit cards are required to be on file for all auto payment services.
Should it become necessary for us to utilize the services of an outside collection agency in order to collect the amounts that are owed, you will be liable for agency/attorney fees.
Flourish Wellness Center is required by State and Federal Law to maintain the privacy of your health information and inform you about our privacy practices. We remain HIPAA compliant and adhere to all State and Federal Law for your safety and health. If you are a new patient please plan to arrive 10-15 minutes before your appointment to fill out our "Notice Of Privacy Practices" document.
For insurance based appointments only: Due to extreme reductions in insurance allowables, Dr. Yerrington is unable to issue refund payments. All overages are kept as a patient credit balance and can be applied toward future services. Any exceptions to this rule will have to be discussed with Dr. Yerrington directly. There are no refunds to any products or merchandise.
